sahasrākṣastadā bhūtvā vajrapāṇirmahāyaśāḥ.

88.
Then, the pleased Mahadeva, the mighty lord of all worlds, assumed the form of Indra (śakra). Surrounded by all the hosts of gods, he then became the thousand-eyed, thunderbolt-wielding, and very glorious (deity).
