योगवासिष्ठः       yogavāsiṣṭhaḥ - book-6, chapter-128, verse-87

अयं चतुर्बाहुरयं विश्वस्रष्टा चतुर्मुखः ।
अयमेव महादेवः संहर्ता च त्रिलोचनः ॥ ८७ ॥
ayaṃ caturbāhurayaṃ viśvasraṣṭā caturmukhaḥ ,
