tanmukhāt oṃ iti mahān abhūt śabdaḥ mahāmune tataḥ

O great sage (mahāmune), from his mouth came the great sound (śabdaḥ) 'Om'. After that, 'Bhūḥ' appeared, then 'Bhuvaḥ' from that, and then 'Svar' immediately afterwards.
