महानिर्वाण-तन्त्रम्       mahānirvāṇa-tantram - chapter-12, verse-76

सुतमादाय सम्मत्या जनन्या जनकस्य च ।
स्वगोत्रनामान्युल्लिख्य संस्कुर्यात् स्वजनैः सह ॥७६॥
76. sutamādāya sammatyā jananyā janakasya ca ,
svagotranāmānyullikhya saṁskuryāt svajanaiḥ saha.

sva-gotra-nāmāni ullikhya saṃskuryāt svajanaiḥ saha
76. jananyā janakasya ca sammatyā sutam ādāya

76. With the consent of both the mother and the father, taking the adopted son, and mentioning his own gotra and names, he should perform the purificatory rites along with his kinsmen.
